This form is provided to assist the Clay County Utility Authority in efficiently and effectively processing public records requests. Completion of this form is not required to submit a public records request.
Requestors are not required to provide personal information, and public records requests may be submitted anonymously. Requests may also be made in person or by phone.
Please note that fees may be associated with fulfilling public records requests, in accordance with Florida law.
Chapter 668.6076, Florida Statutes:
“Under Florida law, email addresses are public records. If you do not want your email address released in response to a public records request, do not send electronic mail to this entity. Instead, contact this office by phone or in writing.”
